What do you send to a college along with a high school transcript?

I've already submitted an online application and my test scores, now I just need to mail in a copy of my high school transcript. What do I include with it? I'm assuming a sheet with some personal information or something so they know who it is?What do you send to a college along with a high school transcript?
If you took any AP classes, and your scores are good, have your school send your scores. You might also want to submit teacher recommendations, an essay if your school accepts one, and an activities resume. An activities resume is a list of all the jobs, community service, and extra curricular activies you took that will not be on the transcript.What do you send to a college along with a high school transcript?
Usually, the college needs a copy of your official transcript. That's a signed/stamped transcript that is in a sealed, unopened envelope - it's not just a photocopy of your college transcript. The official transcript needs to be mailed directly from your high school to the college, without going through your hands.





Many colleges have a form that you print and fill out, and that you hand to your guidance counsellor. She will send that form in with your transcript.
Usually the transcript is sent directly by the high school. Most colleges will not accept transcripts which are sent by the applicant.
The transcript will have your name on it.
