How would I write a transcript request to a college?

Last year i attended a college and i need to write them a letter to request my transcript from last year. I know the basics and stuff of what i have to include but what format or how should i write it?

There is no need to get to formal. Simply state you need a transcript sent to: info


and include (in a list is fine), the info they need.


make sure there is no charge; or you will need to include a check.


Short %26amp; sweet is the easiest because they will be able to pick out the info much easier that way.
