Call/Email the admissions office and ask. To get the phone number for the admissions office at each school, go to that schools website and type admissions in the search box. At the bottom of the admissions page it should have a phone number. There website might also have the information you are looking for. That is the easiest way. Also, the application should say exactly what needs to be submitted.

usually you're schools registrar knows what needs to be sent to colleges as a standard. If the school you want to apply to needs any other info, they usually put out a list of documents that are missing with your application. They do try to help so don't stress too much.
